PRIZES: Two (2) prizes will be awarded through the WorkCompDoctors.com Sweepstakes drawing. The prizes includes one iPad and one LCDTV. The winners will be drawn from business cards voluntarily left in a bowl at the CAAA convention at WorkCompDoctors.com booth in Las Vegas at the Mandalay Bay Resort and Casino.
Winners are responsible for all local, county, state and federal taxes on prizes based on the value of the prizes set forth in these Official Rules.
ELIGIBILITY/ENTRIES: Sweepstakes is open to all U.S. residents, who are U.S. citizens or legal immigrants attending the California Applicants' Attorney Association Winter Convention in San Diego at the Sheraton Hotel and Marina. Participants must be at least 18 years of age to qualify for prize. Void where prohibited / restricted by law. All federal, state and local laws apply. Paying and reporting taxes (if any) are the winner's sole responsibility. Prize may not be substituted, transferred or exchanged, except at the sole discretion of WorkCompDoctors.com. All submissions become the property of WorkCompDoctors.com. Employees and the immediate family members of WorkCompDoctors.com, its parent company and affiliated companies and their immediate families (dependents under IRS definitions) are not eligible to participate.
The Sweepstakes begins at 11:01am Pacific time on June 30, 2011 and ends at 11:00 am Pacific time on July 2, 2011. By entering this Sweepstakes, entrants agree to be bound by these Official Rules. Limit one entry per person via business card. The odds of winning are based upon the number of eligible entries received.
By entering this Sweepstakes, potential winners give their express permission to be contacted by sponsor by telephone, email and/or postal mail. If entrant chooses not to receive subsequent email promotions from WorkCompDoctors.com, entrant may unsubscribe at any time by following unsubscribe instructions on promotional emails received from WorkCompDoctors.com.
All entries become the property of Sponsor and will not be returned. By accepting the prize, winner consents to WorkCompDoctors.com to the use of their name, voice, likeness and photograph for advertising and promotional purposes on-line and in print, or in any other media worldwide in connection with the Sweepstakes, without limitation and without additional compensation. Further, by accepting a prize, winner agrees to not assert any claims and agrees to release WorkCompDoctors.com, its owners, officers, employees, parent company and affiliated companies and third party prize contributors/suppliers from any and all liabilities, injuries or damages sustained in connection with the award or use of the prize(s). Prizes are non-refundable and are subject to exchange.
LIMITATIONS of LIABILITY: Released Parties are not responsible for lost, late, misdirected, incomplete entries or for any computer, on-line or technical malfunctions, delays or human errors that occur in the processing, transmission or receipt of entries, or for inaccurate transcription of entry information, or for entries that are misdirected, garbled, lost or delayed by computer transmission. If, for any reason, the Sweepstakes is not capable of being conducted as planned, including but not limited to infection by computer virus, tampering, unauthorized intervention, fraud, technical failures, or other causes beyond the control of the Sponsor, Sponsor reserves the right at its sole discretion to cancel, terminate, modify or suspend the Sweepstakes and award the prize from all eligible entries received to such date. RESTRICTIONS: Limit one (1) entry per person via business card entered. Sponsor reserves the right to substitute a prize of their choice if the advertised prize is unavailable for any reason. All taxes, fees, and surcharges on the prize and all meals, gratuities, spending money, and all other expenses not specified herein are the sole responsibility of the winner. Prize is not transferable, assignable or redeemable for cash. No prize substitutions are available. There is no cash substitute for prize if winners cannot claim or take prize except at the sole discretion of the Sponsor.
AWARDING PRIZE/WINNER NOTIFICATION: Two (2) winners will be randomly selected from all eligible entries received during the convention and will be notified by phone, email or both on or about July 31, 2011. The prize will be awarded if properly claimed. Contacted winners must respond by phone or email and accept the prize won to redeem it. If prize is not claimed by August 15, 2011 the prize will be forfeited and another winner will be selected by random drawing. Selected winners have 15 days to accept prize once notified via phone, email or both. If sweepstakes sponsor does not receive prize acceptance from the selected winner within the 15-day time period, the selected winner's prize will be withdrawn. In that case, the sponsor will be drawing a new winner. By entering this Sweepstakes, all entrants agree that WorkCompDoctors.com has the sole right to decide all matters and disputes that arising from the Sweepstakes and their determination of the winner is final, binding and conclusive.
This Sweepstakes is subject to all applicable laws and regulations and is void outside of the United States, and where prohibited or restricted by law, including without limitation, in jurisdictions requiring registration, filing, trust account, or posting of bond, or any other requirements or disclosures that are not expressly satisfied by these Official Rules.
